REGISTRATION-WAIVER FORM:
Attached is a registration and waiver form for the homeschool hockey program. It is necessary to sign the waiver before players can receive volunteer instruction.
Since this is a pay-as-you-go program offered by the Hopkins Pavilion, it is not necessary to complete the registration form, but we ask you to do so in order that we can keep a contact list of active participants. Hopkins will charge $3 for kids and $4 for adults each session. There is no additional fee.
